To participate in SCOPE, an office must complete an Application comprised of two parts: Demographics and Questionnaire. Demographics collects basic information about the office, such as name, address, number of office personnel and types of procedures performed. The Questionnaire collects information about patient safety concepts and techniques in the office, referred to as Elements. Some Elements are labeled “Critical.” One hundred percent of these Critical Elements and 80% of remaining relevant Elements must be met before a Questionnaire is submitted.

The Questionnaire must be completed online and can be accomplished in multiple sessions. Please do not send any patient health information and/or documents that do not have all patient identifiers completely removed. In addition, for peer review documentation, such as peer review meetings, please assure that all provider and patient specific information is de-identified. If you are unsure whether the information you are sending contains patient health information, please contact us.
Supporting documentation that can be submitted with the Questionnaire includes office policies and procedures, employee manuals, training materials, office meeting schedules and minutes, personnel records, certification cards, patient handouts and educational materials.
A remote or on-site office review will be scheduled only once a Questionnaire is complete and all required supporting documents are submitted. On-site reviews are selected at random.
