The Application Process

Applying

To participate in SCOPE, an office must complete an Application comprised of two parts:  Demographics and Questionnaire. Demographics collects basic information about the office, such as name, address, number of office personnel and types of procedures performed. The Questionnaire collects information about patient safety concepts and techniques in the office, referred to as Elements. Some Elements are labeled “Critical.” One hundred percent of these Critical Elements and 80% of remaining relevant Elements must be met before a Questionnaire is submitted.

The Questionnaire is divided into five sections that must be completed by all offices:

  • Office Management and Administration
  • Documentation and Reporting
  • Medication Safety
  • Office-Based Surgical Procedures
  • Equipment

The Questionnaire must be completed online and can be accomplished in multiple sessions. Please do not send any patient health information and/or documents that do not have all patient identifiers completely removed. In addition, for peer review documentation, such as peer review meetings, please assure that all provider and patient specific information is de-identified. If you are unsure whether the information you are sending contains patient health information, please contact us.

Supporting documentation that can be submitted with the Questionnaire includes office policies and procedures, employee manuals, training materials, office meeting schedules and minutes, personnel records, certification cards, patient handouts and educational materials.

A remote or on-site office review will be scheduled only once a Questionnaire is complete and all required supporting documents are submitted. On-site reviews are selected at random. 

How to Apply for SCOPE Certification

How to apply:

  1. Request a PDF sample of the full application to be sure you are ready to apply.
  2. Create or log in to your SCOPE account.
  3. Complete the Demographics portion of the Application. 
  4. Request the SCOPE participation agreement. You can view a sample agreement here.  A signed participation agreement and 25% deposit must be received by ACOG before moving beyond the Demographics portion of the Application.  To request a formal agreement for your office, please contact us at scope@acog.org.
  5. Complete the Questionnaire and upload any required supplemental documentation. This can be accomplished in multiple sessions. 
  6. When you feel the Questionnaire is complete, click "SUBMIT" and send the remaining program fee balance payment. Full payment must be received before ACOG will review your Application.
  7. Once ACOG has reviewed your Application, you will receive an e-mail if additional information is required or if the office is randomly selected for an on-site review.  
  8. Following the remote or on-site review, you will receive an email once the report and decision regarding certification are available on your account.
